Player Medical Information

Player's Medical information MUST be made available to the Team Manager/Safety Person.

CRMHA will be asking all Team Managers to provide the following Hockey Canada Medical Information form to parents of every player on their teams.

It is expected that these forms are to be completed in a timely fashion, to ensure that the players' health and safety concerns are made available to Team Officials and the CRMHA.

As well, we will follow the Hockey Canada Privacy Policy with regards to your child's information.
